When is a Dallas Breast Reduction Medically Necessary?
October 13, 2011Women with large breasts (macromastia) often suffer significant pain and discomfort from their large breasts. Dallas breast reduction is an effective treatment for symptoms related to large breasts. In addition to providing physical comfort, Dallas breast reduction can also improve the appearance of the breasts by performing a simultaneous breast lift.
Symptoms of macromastia often include severe neck, shoulder and upper back pain, painful grooves from their bra straps, and rashes, wounds and infections (intertrigo) within the folds of their breasts. Although Dallas breast reduction has been demonstrated to alleviate symptoms associated with large breasts and improve quality-of-life, the amount of tissue required for excision to provide medical benefit has yet to be demonstrated. It is not uncommon for me to believe that a Dallas breast reduction will help my patient, only to have the patient’s insurance company disagree.
Breast reduction surgery is considered by the American Society of Plastic Surgeons to be medically necessary when symptoms of back pain, neck pain, shoulder pain, intertrigo, breast pain, and numbness of the extremities are documented and related to the patient’s large breasts. The recommendations of the ASPS are based on outcomes studies that essentially demonstrate no correlation between amount of breast tissue excised and relief of preoperative symptoms. Unfortunately, when symptoms are self-reported and subjective, it is difficult to determine medical necessity as, often, breast reduction is a cosmetic procedure. As a result insurance companies usually require extensive documentation of symptoms, as well clinical notes documenting failure of conservative treatment, including supportive bras, NSAIDs such as ibuprofen, physical therapy and chiropractic treatments. In addition, most insurance companies have a minimum tissue excision requirement that must be met to demonstrate medical necessity for Dallas breast reduction surgery.

Dallas Silicone Breast Implants & MRI – What you need to know
September 12, 2011When the Food & Drug Administration (FDA) approved silicone breast implants for cosmetic breast augmentation in 2006, it did so with recommendations regarding screening for breast implant rupture. Specifically, the FDA recommended that patients get a screening magnetic resonance imaging test (MRI) for breast implant rupture three years after breast augmentation and then every two years thereafter.
The reason for this recommendation is that, often, ruptured silicone breast implants are not detectable on physical examination. In these cases, MRI is the most sensitive method for detecting silicone breast implant rupture. Unfortunately, though, MRI can be an expensive exam and the costs usually are not covered by third-party insurance companies. Furthermore, the significance of a previously undetected silicone breast implant rupture is not clear, as it is not certain that the risks associated with ruptured implants is greater than the risks of the operation needed to take them out.
Last month, an expert FDA panel spent two days re-visiting the safety of silicone breast implants. The FDA’s chief scientist for devices, Dr. William Maisel, reaffirmed the safety of silicone breast implants. The panel agreed that patients no longer need to be told that they should get an MRI three years after getting silicone breast implants and every two years thereafter. The panel confirmed that MRI remains the gold standard for screening for silent breast implant rupture. This recommendation, however, has yet to be adopted by the FDA at large.
So what’s the conclusion? For now, the FDA recommendations from screening MRI three years after silicone breast augmentation and every two years thereafter remain intact. This guideline, though is a recommendation – not a requirement. Given the recommendations of the FDA’s expert panel, every patient should determine their own comfort level for living with a silent rupture, and make their own decision based on this.

Dallas Facelift – what do I need to know?
August 7, 2011Facelift remains the gold standard for facial rejuvenation. Although many great noninvasive (Juvederm, Restylane. Botox, Ulthera, Thermage, etc.) and minimally invasive (Fraxel laser, chemical peel, dermabrasion) options exist for facial rejuvenation, none of these treatments can achieve the same level of improvement that are achievable with a facelift. So, assuming you accept this, and are prepared to undergo surgery, what do you need to know before your facelift at Dallas Plastic Surgery Institute?
First, don’t rush your recovery. Vitamin supplements, including Bromelain Sulfate and Arnica Montana, can help bruising and swelling resolve more quickly after surgery, but don’t expect miracles! You should expect bruising and swelling for at least 10-14 days after surgery and, in some cases, up to three weeks. Planning to have surgery a few days before a big vacation or an important work meeting is undoubtedly a set up for a problem.
Two, make sure your spouse or significant other is supportive of surgery. The swelling and bruising can be considerable after surgery. Having a demonstrably angry or agitated spouse can only add to natural anxiety that you will have immediately after surgery. In addition, it’s incredibly important to have someone trusted help you do relatively simple things, such as get out-of-bed or bathe.
Three, make sure you fill all prescriptions and have read your postoperative instructions prior to surgery. Dallas facelift surgery is commonly combined with other procedures, including eyelid lift (blepharoplasty), laser resurfacing, and brow lift. A variety of different treatments will be needed after surgery and it’s important that you have a thorough understanding prior to surgery.

Cutting back on Botox does not compromise results after two-years of regular treatments
July 31, 2011A new study from scientists at Oregon Health & Science University confirms what many patients have suspected for some time: After undergoing regular treatments with Botox, patient can space out the length of time between treatments without sacrificing results. In the study, 50 women between the ages of 30 and 50 underwent Botox injections every 4 months for 2 years. After that time, the patients only underwent injections every 6 months without sacrificing results. Furthermore, the study showed, as many patients suspect, that Botox is actually effective in preventing wrinkles from forming.
Treatments with Botox are one of the most popular cosmetic procedures worldwide, with millions of patients treated annually. It is FDA-approved for treatment of glabellar frown lines (the “elevens” or wrinkles between the eyebrows) and is commonly used off-label for treatment of forehead wrinkles and crow’s feet. This study demonstrates that patient’s undergoing chronic treatment can anticipate lowering the yearly cost of treatment after two years of regular treatments.

Important Considerations Before Cosmetic Surgery
July 29, 2011Many people ask preoperatively what they can do to minimize the chances of problems after surgery. Although the possibility of complications always exists, there are very things that you can do to minimize complications and to speed up the recovery process.
First, it is very important to stop using tobacco. Many clinical studies have shown that tobacco usage significantly impairs wound healing and can lead to healing complications. Stopping all tobacco use at least 4-6 weeks before surgery will lessen the chances of wound healing complication and infection. Although quitting tobacco is not easy, it is incredibly import to optimize the safety of surgery.
Second, it is important to eat well. A diet high in protein improves the ability of the body to heal large surgical incisions. The interval of time just before surgery is not the time to diet and to become malnourished, as it will increase your risk of complications. In addition to adequate protein intake, a well-balanced diet supplemented with a multivitamin can be helpful.
Third, it is important to be in a good overall state of health. What does this mean exactly? It means that, if you have diabetes, make sure your blood sugars are well controlled. If have heart disease, make sure your blood pressure and cholesterol levels are optimizes preoperatively. This may seem self evident, but having a major medical complication after elective surgery can not only be harmful to your health, but it could also lead to significant complications with healing after surgery.
Fourth, it is important that you have a support structure intact that allows you to recover after surgery. What I mean by this is that patients who do not have help after surgery are oftentimes forced to do too much. Straining and excessive movement can lead to increased swelling and to other potential complications after surgery. In addition, if you do not give yourself enough time to recover from surgery and have to return to work prematurely, you can increase the risks of complications as well.

Nonsurgical nose job – case study
July 28, 2011Here’s an interesting alternative to surgical rhinoplasty. This patient presented to my office a little more than a year after undergoing rhinoplasty (nasal surgery) in another city. He was unhappy with a depression along the left side of his nose, which, on my exam, seemed to be deficient or absent nasal cartilage. He was reluctant to undergo repeat surgery, but wanted to learn about nonsurgical options.
I recommended treatment with Juvederm, a hyaluronic acid filler. He was treated with one syringe of product. He is seen before the injection (left), immediately after the injection (center) and one week after the injection (right). The results are not permanent, but commonly last one year or more.
