			<content:encoded><![CDATA[<p>The official 2011 statistics of cosmetic plastic surgery procedures performed in the United States were just released by the American Society of Plastic Surgeons. Although not a traditional indicator of the state of the economy, just as unemployment rates and the stock market saw improvement in 2011, so did the numbers of cosmetic surgery cases performed in the United States. Nearly 14 million procedures were performed last year, up 5% from the year before.</p>
<p>The top 5 procedures performed in 2011 were:</p>
<ul>
<li>Breast augmentation (307,000 procedures)</li>
<li>Rhinoplasty (nose reshaping; 244,000 procedures)</li>
<li>Liposuction (205,000 procedures)</li>
<li>Eyelid surgery (196,000 procedures)</li>
<li>Facelift (119,000 procedures)</li>
</ul>
<p>Minimally invasive procedures also saw an increase in 2011, up 6 percent. The most commonly performed minimally invasive procedures were Botulinum type A toxin injections (e.g. Botox, Dysport, etc.), soft tissue fillers (e.g. Juvederm, Restylane, Radiesse, Sculptra, etc.), chemical peel, laser hair removal and microdermabrasion.</p>

			<content:encoded><![CDATA[<p>Nicotine and tobacco addiction is a major problem for any patient looking to have elective cosmetic surgery. As I tell all my patients, the risks of healing complications are significantly higher in smokers, particularly for operations that require lifting up of large amounts of skin (such as facelift, breast lift, tummy tuck, etc.). Patients often time seek help to quit smoking before surgery, turning to such treatments as electronic cigarettes (e-cigarette), behavior modification therapies, and medications such as Chantix or Zyban.</p>
<p>Treatments that have been frequently used as well are nicotine replacement therapies, including nicotine patches and gums. In a study published yesterday, researchers from Harvard found that two-thirds of patients who quit smoking resumed smoking within four years, even when used in conjunction with behavioral modification. The study looked at nearly 800 patients and found the same relapse rate in people who used no smoking-cessation aids.</p>
<p>While the results of the study are certainly discouraging for patients looking to quit smoking permanently, the do not contradict the very real short term effects these products can have. Patients may be better off utilizing other forms of therapy, particularly medications such as Chantix and Zyban.</p>

			<content:encoded><![CDATA[<p>Medicis Aesthetics, the manufacturer of Restylane, announced on October 11 that Restylane has now received FDA approval to be used for lip augmentation. Although Restylane has been used extensively for lip augmentation, doing so was considered an off-label indication. Restylane, a hyaluronic acid dermal filler,  was first approved in 2005 for mid to deep dermal injection for the  correction of moderate to severe facial wrinkles and folds, and has been  used off-label for lip augmentation.</p>
<p>An FDA advisory  panel had previously voted 6-0 with 1 abstention that the benefits of using Restylane  as an injection for lip augmentation outweighed its risks, and  that the filler was safe and effective for this indication. The panel reviewed the results of a  study of 135 patients who received Restylane  and 45 patients with no treatment. Almost all patients (99%) experienced adverse events, which included  bruising, redness,  swelling, pain, tenderness, itching, and skin exfoliation. Of those who  were treated, 15% experienced adverse  events (typically swelling and tenderness) that lasted more than 15  days.</p>

			<content:encoded><![CDATA[<p>Women with large breasts (macromastia) often suffer significant pain and discomfort from their large breasts. <a href="http://drjdallasplasticsurgeon.com/breast/breast-reduction/">Dallas breast reduction</a> is an effective treatment for symptoms related to large breasts. In addition to providing physical comfort, Dallas breast reduction can also improve the appearance of the breasts by performing a simultaneous breast lift.</p>
<p>Symptoms of macromastia often include severe neck, shoulder and upper back pain, painful grooves from their bra straps, and rashes, wounds and infections (intertrigo) within the folds of their breasts. Although Dallas breast reduction has been demonstrated to alleviate symptoms associated with large breasts and improve quality-of-life, the amount of tissue required for excision to provide medical benefit has yet to be demonstrated. It is not uncommon for me to believe that a Dallas breast reduction will help my patient, only to have the patient&#8217;s insurance company disagree.</p>
<p>Breast reduction surgery is considered by the American Society of Plastic Surgeons to be medically necessary when symptoms of back pain, neck pain, shoulder pain, intertrigo, breast pain, and numbness of the extremities are documented and related to the patient&#8217;s large breasts. The recommendations of the ASPS are based on outcomes  studies that essentially demonstrate no correlation  between amount of breast tissue excised and relief of preoperative  symptoms. Unfortunately, when symptoms are self-reported and subjective,  it is difficult to determine medical necessity as, often, breast reduction is a cosmetic procedure. As a result insurance companies usually require extensive documentation of symptoms, as well clinical notes documenting failure of conservative treatment, including supportive bras, NSAIDs such as ibuprofen, physical therapy and chiropractic treatments. In addition, most insurance companies have a minimum tissue excision requirement that must be met to demonstrate medical necessity for Dallas breast reduction surgery.</p>

			<content:encoded><![CDATA[<p>When the Food &amp; Drug Administration (FDA) approved silicone breast implants for cosmetic breast augmentation in 2006, it did so with recommendations regarding screening for breast implant rupture. Specifically, the FDA recommended that patients get a screening magnetic resonance imaging test (MRI) for breast implant rupture three years after breast augmentation and then every two years thereafter.</p>
<p>The reason for this recommendation is that, often, ruptured silicone breast implants are not detectable on physical examination. In these cases, MRI is the most sensitive method for detecting silicone breast implant rupture. Unfortunately, though, MRI can be an expensive exam and the costs usually are not covered by third-party insurance companies. Furthermore, the significance of a previously undetected silicone breast implant rupture is not clear, as it is not certain that the risks associated with ruptured implants is greater than the risks of the operation needed to take them out.</p>
<p>Last month, an expert FDA panel spent two days re-visiting the safety of silicone breast implants. The FDA&#8217;s chief scientist for devices, Dr. William Maisel, reaffirmed the safety of silicone breast implants. The panel agreed that patients no longer need to be told that they should get an MRI three years after getting silicone breast implants and every two years thereafter. The panel confirmed that MRI remains the gold standard for screening for silent breast implant rupture. This recommendation, however, has yet to be adopted by the FDA at large.</p>
<p>So what&#8217;s the conclusion? For now, the FDA recommendations from screening MRI three years after silicone breast augmentation and every two years thereafter remain intact. This guideline, though is a recommendation &#8211; not a requirement. Given the recommendations of the FDA&#8217;s expert panel, every patient should determine their own comfort level for living with a silent rupture, and make their own decision based on this.</p>
